HealthLandscape is an enterprise of the American Academy of Family Physicians (AAFP) specializing in data analysis and visualization, particularly through web-based mapping tools.

HealthLandscape.org - is an interactive web atlas that brings together various sources of health, socio-economic and environmental information in a convenient, central location to help answer questions about and improve health and healthcare. HealthLandscape.org serves as the platform for all of HealthLandscape's mapping tools, which are listed below.


HEALTH WORKFORCE & GRADUATE MEDICAL EDUCATION APPLICATIONS
HealthLandscape has created a number of mapping tools that display the distribution of health workforce and the geographic reach of graduate medical education. More information about the first four tools can be found on the Robert Graham Center website.

HealthWorkforce Mapper - This tool illustrates the distribution of healthcare workforce and allows for comparisons of physicians and non-physicians.


SITE EXPLORERS
HealthLandscape's Site Explorer tool allows users to evaluate performance across multiple sites. Sites are colored red, yellow, or green based on relevant cri teria, providing users with a quick visualization of site performance and allowing users to quickly pinpoint high-performing or low-performing sites. Examples of HealthLandscape Site Explorers are listed below and include the ACO Quality Explorer, the Readmissions Explorer, and the Nursing Home Star Ratings Explorer.

The Accountable Care Organization (ACO) Quality Explorer is an interactive mapping tool that presents data for 211 ACOs taking part in the Medicare Shared Savings Program (MSSP). The ACO Explorer allows users to visualize 33 quality metrics across five domains, which are compared against benchmarks set by the Centers for Medicare and Medicaid (CMS). Users immediately see whether an ACO is above, within range, or below quality benchmarks, designated by green (above), yellow (within range), or red (below) circles displayed on a map. When users click on a specific site, fly-out menus appear that give more detail about the measures that make up each of the five domains, allowing easy comparison of quality measures across ACOs. In addition, the ACO Quality Explorer allows for statistical analyses that enable users to explore the relationships between ACO quality metrics and population health indicators at the county level.

The Hospital Readmissions Explorer is an interactive mapping tool that allows users to quickly compare readmissions for U.S. hospitals. Hospitals are colored red, yellow, or green based on whether hospitals have readmissions worse, no different, or better than national average for all-cause readmissions and for the individual diagnosis of Heart Failure, Heart Attack (AMI), Pneumonia, COPD, Stroke, and Hip/Knee Replacements. Mortality data have also been added which allows users to explore readmissions and mortality rates. Users can also filter by whether a hospital was fined for excess readmissions in 2015, received a penalty as part of the Hospital Value-Based Purchasing (HVBP) Program, and by State. In addition, users can use the Stats tool to perform statistical analyses to explore relationships between hospital readmissions and mortality rates and population health indicators for the county in which the hospital is located.

The Nursing Home Star Ratings Explorer is a user-friendly spatial visualization tool for Nursing Home Compare data for U.S. nursing homes. Each nursing home is displayed as a red, yellow, or green point on the map based on its overall quality star-rating, allowing users to quickly compare the quality of neighboring nursing homes. When a user moves their mouse over a point, the nursing home name, address, and the star-ratings for the nursing home's staffing, health inspections, quality measures, and overall ratings are displayed in windows on the screen. Users are able to filter the selection of nursing homes displayed on the map by ownership, provider type, or state, and have the option to view the data displayed on the map in a table format. In the Stats tab, users can examine the correlation between nursing home characteristics and community demographic and health data for the county in which the nursing home is located. The Nursing Home Compare Explorer is a very useful tool for quickly comparing multiple nursing homes and provides an important spatial visualization component for the Nursing Home Compare data.


POPULATION INDICATORS TOOLS
The Population Indicators Tool allows users to set thresholds and overlay multiple indicators to identify areas of need or bright spots. Examples of HealthLandscape Population Indicators Tools are listed below and include the Children's Health and Education Mapping Tool, the Population Health Mapper, and the Social Determinants of Health Mapper.

Project 500 Cities Mapping Tool was developed using data released through the 500 Cities project, a project created by the Centers for Disease Control and Prevention (CDC), along with the Robert Wood Johnson Foundation and the CDC Foundation. Its purpose is to allow cities and local health departments to gain a better understanding of the health issues and geographic distribution of health measures in their municipal boundaries. With the Project 500 Cities Mapping Tool, users can map small area estimates for chronic disease risk factors, health outcomes, and clinical preventive services at the census tract level for the largest 500 cities across the U.S. (To ensure the inclusion of all states, 3 cities from Vermont, West Virginia, and Wyoming were included in the list of 500.) The Children's Health and Education Mapping Tool allows users to harness the power of geographic information systems (GIS) for data-driven decision-making. County-level data can be searched, mapped, downloaded, and compared, and high need areas can be identified. Schools and school-based health centers (SBHCs) can be mapped, filtered, and key characteristics can be displayed. This tool is the result of a collaboration between the School-Based Health Alliance and HealthLandscape.

The Population Health Mapper The Centers for Disease Control and Prevention (CDC) created a resource guide of the most frequently recommended health outcomes and determinants, the Community Health Assessment for Population Health Improvement. Using this guide as a framework, HealthLandscape created the Population Health Mapper, which includes the majority of the Health Outcome and Health Determinant Metrics identified in the report at the county level. The Mapper allows users to select metrics from seven categories (Mortality, Morbidity, Health Care, Health Behaviors, Demographics, Social Environment, and Physical Activity) and use a slider bar to set thresholds. By default, thresholds are set at values that represent national benchmarks. The tool will highlight those counties that are outside of the national benchmark, or will incrementally shade or remove counties depending on how the user modifies the thresholds for selected indicators. Darker gradations of color will indicate which counties are outside of the established thresholds for multiple indicators. Users can also view a histogram that shows the number of counties outside of thresholds by the number of indicators, allowing users to quickly filter by the number of indicators that are outside of the established thresholds.


COMMUNITY DATA PORTALS
Community Data Portals allow users to visualize data using charts, graphs, and maps across a variety of topics. Examples of HealthLandscape Community Data Portals are listed below and include the Our Kids Network Data Portal and the Appalachia Data Portal.

The Medicare Data Portal is a powerful tool for visualizing and understanding Medicare data, giving users easy access to an array of hospital referral region (HRR) and county-level Medicare indicators that can be displayed as maps, tables, bar charts, or trend graphs. Users can also examine the relationship between two health indicators by creating side-by-side and comparison maps. The Medicare Data Portal contains a wide variety of Medicare data, including multiple years of indicators from the Geographic Variation in Medicare Spending and Utilization database and the Chronic Conditions Warehouse from the Centers for Medicare and Medicaid Services. These data have been supplemented with Medicare indicators from the Dartmouth Atlas, including data related to Medicare Reimbursements, Primary Care Access & Quality, Post-Discharge Events, Hospital Discharges, Prescription Drug Use, and End of Life Care.

The Appalachia Data Portal is an online data visualization tool for exploring demographic, education, income, and health disparities for the 420 counties in the Appalachian region. The data included in the Appalachia Data Portal come from a variety of sources, including the American Community Survey, the Appalachian Regional Commission, the Robert Wood Johnson County Health Rankings, and the Centers for Medicare & Medicaid. The tool allows users to visualize economic, demographic, and other types of data for the Appalachian region using maps, graphs, and trend charts. Users also have the ability to examine the relationship between two indicators (for example, Diabetes and Poverty) with side-by-side maps and a comparison tool that uses percentiles to visualize the relationship between variables. The Appalachia Data Portal provides an array of tools for exploring population health throughout the Appalachian region, and can be useful for identifying health disparities and bright spots within the region.

HealthLandscape Australia displays a range of data relating to measures of health outcomes, healthcare workforce, sociodemographics, administrative boundaries and places of interest.
