116th Congressional District Boundaries

Description
Displays the 435 Congressional Districts from which people are elected to the U.S. House of Representatives, as defined by the U.S. Census Bureau. After the apportionment of congressional seats among the States based on census population counts, each State is responsible for establishing congressional districts for the purpose of electing representatives. Each congressional district is to be as equal in population to all other congressional districts in a state as practicable. The 116th Congress is seated from January 2019 to December 2020. The TIGER/Line shapefiles for the District of Columbia, Puerto Rico, and the Island Areas (American Samoa, Guam, the Commonwealth of the Northern Mariana Islands, and the U.S. Virgin Islands) each contain a single record for the non-voting delegate district in these areas. The boundaries of all other congressional districts are provided to the Census Bureau through the Redistricting Data Program (RDP).

Description
Rankings calculated by the Health Resources and Services Administration (HRSA) based on performance after adjusting for factors including minority patient population, uninsured patient population, and special patient population. Body Mass Index (BMI) Screening and Follow-Up Plan

Description
Percentage of medical patients aged 18 years and older with a documented BMI during the most recent visit or within the six months prior to that visit. The patient is counted if they had a BMI outside of normal parameters and a follow-up plan was documented during the visit or during the previous six months of the visit with the BMI outside of normal parameters. The Adult Weight Screening and Follow-Up Plan measure was revised in 2016 and is not comparable to prior years. This measure was aligned with a specific clinical measure, CMS69v6, used by the Centers for Medicare and Medicaid Services. Description

The user can scroll through to find the organization name and address.

Drug Poisoning Mortality

Description
Display is based on the estimated mortality rate (age-adjusted rate of deaths per 100,000 standard population) by drug poisoning. Drug-poisoning deaths are defined as having ICD–10 underlying cause-of-death codes X40–X44 (unintentional), X60–X64 (suicide), X85 (homicide), or Y10–Y14 (undetermined intent).

When the cursor is over the symbol on the map, the site name and address will be available in the Information Card. Remember there is no patient information in the UDS Mapper specific to the Facility and Point HPSAs.

Health Center Administrative Locations

Description
Displays the administrative locations of all the organizations that are Health Center Program (HCP) awardees or look-alikes. In the UDS Mapper, HCP awardees are displayed as dots and HCP look-alikes are displayed as triangles. These dots and triangles are larger than the ones displayed for the health center service delivery sites. When the cursor is over the dot or triangle on the map, more information is available about the organization in the Information Card. The user can scroll through to find the organization address, type of health center funding or if they are a look-alike, whether they are a Patient Centered Medical Home that has been recognized by BPHC, and if they have an EHR fully implemented in all sites and all providers using it. Information from the UDS about patient demographics, patient characteristics, quality, costs, and services is also included if the Info Card Deck button is activated.

Health Center Service Delivery Sites

Description
Displays the locations where Health Center Program grantees and look-alikes provide services. In the UDS Mapper, HCP grantees are displayed as dots and HCP look-alikes are displayed as triangles. These dots and triangles are smaller than the ones displayed for the administrative locations. When the cursor is over the dot or triangle on the map, more information is available about the site in the Information Card including the site name, organization that it is a part of, site address, and operations schedule. This information cannot be changed in the UDS Mapper; instead, the information must be corrected at BPHC.

Health Centers With a DATA Waived Provider

Description
Displays Health Center Program awardees with a physician, physician assistant, and/or nurse practitioner providing Medication-Assisted Treatment (MAT) for opioid use disorders. In the UDS Mapper, Health Centers with a DATA Waived Provider are displayed as black dots within yellow circles. When a dot is clicked on the map, more information is available about the organization in the Information Card. The user can scroll through to find the organization name and address, the number of DATA waived providers, and the number of patients receiving services from a DATA waived provider.

Description
Displays the health centers with the greatest number of patients out of all the health centers reporting at least 11 patients who live in the ZCTA). Up to five health centers are listed for the each ZCTA. The health centers are listed in order of their Share of Patients from highest to lowest.

Hospital Referral Regions (HRRs)

Description
Displays HRRs which represent regional health care markets for tertiary medical care that generally requires the services of a major referral center. The regions were defined by determining where patients were referred for major cardiovascular surgical procedures and for neurosurgery. Each hospital service area (HSA) was examined to determine where most of its residents went for these services. The result was the aggregation of the 3,436 hospital service areas into 306 HRRs. Each HRR has at least one city where both major cardiovascular surgical procedures and neurosurgery are performed.
