Screening for Clinical Depression and Follow-up Plan

Description
Percentage of patients aged 12 years and older screened for clinical depression on the date of the visit using an age-appropriate standardized depression screening tool, and, if screening is positive, for whom a follow-up plan is documented on the date of the positive screen. The Depression Screening and Follow-up measure was revised in 2016 and is not comparable to prior years. This measure was aligned with a specific clinical measure, CMS2v5, used by the Centers for Medicare and Medicaid Services. Tobacco Use: Screening and Cessation Intervention

Description
Percentage of medical patients aged 18 and older who were screened for tobacco use one or more times within 24 months and who received cessation counseling intervention if identified as a tobacco user. The Tobacco Use measure was revised in 2016 and is not comparable to prior years. This measure was aligned with a specific clinical measure, CMS138v4, used by the Centers for Medicare and Medicaid Services. This number is inputted by the user. It should represent the total number of patients that will be served by the site or sites under analysis who are not currently health center patients. This can be for future health center sites or for existing providers in the area who are not health centers. Remember all health center patients from the user-selected analysis area are already counted in this table, therefore the user must estimate how many of the total patients for the site under analysis were already counted as health center patients and subtract that from the total patients to be seen at the site under analysis to avoid duplicate count of patients.
